To answer the OP's question--to the best of my ability and current knowledge: When I was leveling my Dwarf last week, and this may have been prior to YomanROM's reference to the 6/15 maintenance, I received a pair of Level 15 Elite Skill Training certificates in the in-game mail. I went to the Elite Skill Trainer in the starting area, received the level 15 Elite for Warlock, changed classes, then went to him again and received the level 15 elite for Mage.
After leaving the starting area and later returning to get my third class (Champion--Dwarves can only receive their third class in the Dwarven starting area, class trainers in Varanas cannot help them), I later went to Heffner Camp and did several of the Old Bag quests to get my level 20, 25, and 30 elite skills for all three classes. As tmblake has said, only one certificate seems to be needed to gain the elite skills for the given certificate level. This may or may not be a bug. If it is, it will likely be fixed at a later date.
I do not know what the precise sequence is now for getting the level 15 certificates, but I presume that you still get them in the starting zone, and they arrive in your in-game mail, and need to be redeemed in the starting area. If anyone has current information which differs from this, I'd appreciate the correction.
