Quick update/addition for you:

Mind is now called Illusion, and the chicken boss (forget his name -- Raisha or something like that) in the Ystra Labyrinth drops the V pretty regularly.

Mystic V is dropped in Ice Dwarf Heroic chests, not in clops.
And Sage V is dropped from Gorn

Didn't see these mentioned in the OP:

Force V drops off the chicken boss in the Ystra Labyrinth. Barbarian V drops off the Rune Leader in the labyrinth as well.

Yeah it's 30 Str/30 Stam, but it never drops clean. It's a great mod to match to a dirty on an accessory though! This mod is even good for casters considering it gives 30 stam.

Fang V can be found on the ant kings ... they drop a knife of some sort that has it on there, not sure if it's clean or not, may be random like the rest, but yea... I'll be going there later to farm some of those up ^^
